Dues Information & Membership Applications
To become a member, please download and complete the membership application and return it with payment to the address or fax number on the application form. Once the Executive Office has processed your membership, your confirmation letter and password for the members' only section of the website will be mailed to you.

Membership Levels
Rehabilitation Facility/Provider
Dues amounts are based on a sliding scale.
Group
$1000 (Non Service Provider) Includes organizations, government offices, universities that do not directly provide rehabilitation services.
Individual
$500 Individuals desiring to support the work of the association who are not employed by an eligible Facility/Provider or Group.
Student (enrolled full-time)
$50 Student Members must be enrolled full-time. Please submit a copy of student ID with membership form.
OARF Program Division(s)
- Vocational Rehabilitation
- Industrial Rehabilitation (Occupational, BWC provider)
- Medical Rehabilitation (Inpatient, Outpatient except for Occupational Rehab)
Dues include benefits of one division; additional divisions are $100 each. Benefits include e-publications, quarterly division meetings and voting privileges.
Membership Year
The membership year runs from January 1 to December 31.
