About the Presenter

Chris M. MacDonell is CARF's Managing Director of Medical Rehabilitation & Aging Services units. She has more than 35 years' experience as a provider, administrator, and trainer. Beginning her career as an occupational therapist, Chris became a rehabilitation administrator of a full continuum of programming that included medical rehabilitation and behavioral health services. She worked with a California rehabilitation association assisting medical and vocational providers receive education to enhance service delivery. Prior to coming to CARF in 1991, she was the Executive Director of the California Alliance of Rehabilitation Industries. Since 1991 she has represented CARF at international and national meetings introducing and promoting the concepts of quality oversight and enhancement of services through the CARF accreditation process.

Continuing Education Information

Approval by OT/PT/AT Board for Occupational & Physical Therapists: 5.50 CEUs pending
Approval by Ohio Nurses Association: 6.6 CEs pending.

This activity is being submitted to the Ohio Nurses Association, an accredited approver by the American Nurses Credentialing Center's Commission on Accreditation. Attendees MUST attend entire seminar and complete an evaluation form to be awarded CE credits.
Certificate of attendance will be distributed at conclusion of the seminar for paid participants only.

Fee includes training materials and beverage breaks. Registrations accepted on official registration form only. Photocopies are acceptable. Registrations must be pre-paid and received by October 22, 2008. No purchase orders.

CANCELLATION – SUBSTITUTION POLICY
Cancellations are subject to a $25.00 cancellation fee. Cancellation notices must be sent in writing to the OARF by email to: oarf@sbcglobal.net. No refunds on cancellations; forfeited fees cannot be applied to future meetings due to IRS reporting requirements. Substitution from within the same organization IS permitted in lieu of cancellation. All substitutions must be in writing and accompanied by a completed registration form.
